Semaglutide

Semaglutide is a GLP-1 receptor agonist approved by the FDA as Ozempic (type 2 diabetes) and Wegovy (weight management). It reduces appetite and slows gastric emptying, producing an average of roughly 15% body-weight loss over 68 weeks in clinical trials.

What it is

Semaglutide mimics the gut hormone GLP-1, which is released after eating. It activates GLP-1 receptors in the brain and gut to reduce hunger, slow stomach emptying, and improve insulin response. A fatty-acid side chain lets it bind to albumin in the blood, giving it a one-week half-life so it can be injected weekly. Research-grade semaglutide is sold lyophilized in vials and requires reconstitution, unlike branded pens.

  • STEP 1 trial: 14.9% mean weight loss at 68 weeks vs 2.4% placebo in adults with obesity (PMID 33567185)
  • SELECT trial: 20% reduction in major cardiovascular events in adults with overweight/obesity and existing cardiovascular disease (PMID 37952131)
  • SUSTAIN trials: reductions in HbA1c of 1.5–1.8% in type 2 diabetes
  • Weight regain after discontinuation: roughly two-thirds of lost weight regained within a year (PMID 35441470)

Side effects reported
